Step 1 Define Instructional Goal:


Skills needed: Verbal, Psychomotor Skills, Intellectual Skills


Instructional Goal: Students will be able to perform a Venipuncture by following the steps required for each procedure with a 100% accuracy.


 

Step 2 Define Learning Objectives:


  1. Accurately identify the patient using two forms of identification

Does the name and date of birth on the requisition form match what the patient is telling you, and match what is on the ID wristband?

Has the patient followed orders such as fasting prior to blood draw?

  1. If information does not match, Address the inconsistency first with the requesting physician or health care worker on duty

  2. If given information does match go on to next step

  1. Phlebotomist washes hands and dons a clean pair of gloves

  2. Make sure all supplies are within an easy reach of you, but out of reach of patient (alcohol prep, gauze, tape, and required evacuation tube)

  3. Speak reassuring words to the patient as you prepare them for the draw

  4. Apply the tourniquet

  5. Use your finger to palpate the antecubital fossa - to locate the median cubital vein

Palpation helps determine the blood flow in the vein to gauge the veins size and depth and to estimate it’s tendency to roll

Differentiates veins from arteries (arteries having thicker feeling walls and are more elastic to the touch) 

  1. Use an alcohol prep pad to cleans the site in a fluid circular motion swab from the center to the periphery of the site being careful not to drag the pad past the cleaned field.

  2. Let the area air dry to prevent contamination

  3. Grasp the patient's arm firmly with your hand placing your thumb approximately two inches below the intended puncture site.

  4. With your thumb pull the skin tight over the vein (Maybe here we have an option if you can’t find a vein to return to step 9 on the other arm??

  5. Let the patient know you are about to make the puncture (so patient doesn’t startle and jump)

  6. Position the needle bevel side up and line it up with the vein

  7. Position the needle so it forms a 30 degree surface with the arm

  8. With a single short and firm motion swiftly insert the needle into the vein (would we need something here too if the vein doesn’t work?

  9. Push the evacuated tube onto the needle when the needle enters the vein-As the vein aligns with the needle blood will begin to move

  10. As soon as blood begins to move into the collection tube instruct the patient to open his hand.

  11. Remove the tourniquet from his arm.

  12. Remove the collection tube and gently mix at least 8 times to make sure additives are mixed and incorporated into the blood sample

  13. Engage the safety device on the needle and use the sharps device for proper disposal.

  14. Apply pressure at the needle site immediately after to prevent a hematoma from forming and allow hemostasis to heal the wound
